Once the form is completed, be sure to print your application form and have your parent or guardian sign it before mailing it to our office using the information shown on the right.

read through the lifeworks information packet (and return necessary forms)
After March, Lifeworks will send you an information packet including important forms, packing lists, summer communication information, travel instructions and more.
